EMERGENCIES: In the event of an emergency, I authorize this establishment to immediately seek professional veterinarian attention for my pet (at my expense). I understand that all attempts will be made to contact me in the event of an emergency. We are not responsible for underlying/unknown/previous conditions.
COAT CONDITION: I understand that this establishment puts my pet's comfort above all else.
WE DO NOT DEMATT COATS
The maintenance of your pet is YOUR responsibility. The groomer can only do so much within reason with whatever condition your dog is presented upon their grooming appointment.
I also understand that if my pet is severely matted, it will be shaved down. There is an increased risk for clipper burn or cuts to occur. I understand that all attempts will be made to prevent this, however in many extreme matt conditions, it is unavoidable. I also understand that matted pets take additional time to groom so there will be an additional fee added on to the regular grooming price if my pet's coat is matted.
HEALTH: I understand that grooming can be stressful to some pets and I will inform the groomer if my pet has any medical conditions or any stress related issues prior to grooming. I also understand that it is necessary to have my pet up to date on all vaccinations prior to every grooming.
CANCELLATION POLICY: I understand that if I need to change my appointment time or cancel it, that I must do so at least 2 days before my appointment I will be charged the full amount for my appointment. If I no show for my appointment I will be charged the full amount for my appointment. If I need to reschedule I will do so at least 2 business days before my appointment or I will be charged the full amount for the appointment I am changing. If I reschedule, cancel, or no show more then 3 times in a year, Bone Appetit has the right to refuse continued service for my pet.
HOLD HARMLESS AGREEMENT: By signing this contract I (or my Agent) agree to hold Bone Appetit, it's owners, operators, employees, officers and directors harmless from any damage, loss, or claim arising from any condition of the undersigned pet, either known or unknown up to and including death. It is also further understood and agreed the terms of this agreement can change at any time, without notice, and will overwrite any and all prior signed contracts or releases. It further understood this clause applies to any and all pets groomed.
SIGNATURES:
This contract may be signed electronically or in hard copy. If signed in hard copy, it must be
returned to Bone Appetit for valid record. Electronic signatures count as original for all purposes.
By typing their names as signatures below, both parties agree to the terms and provisions of this
agreement.
